A Few Requirements for Botox Events:

• Each new patient must be examined by our Medical Director Dr. Voci, or nurse
injector prior to being treated.

• Proper records must be completed, including medical history, and consent forms must be signed in advance. Before photos will be taken at the event, with After photos taken at the Medical Spa after 10-14 days later at the follow-up appointment.

• Events are limited to a maximum of six guests (including the host); Contact
information (including names, email addresses and cell phone numbers) for each guest must be submitted seven days in advance of the event for proper consent forms to be sent.

• Your reservation will include the use of our entire, beautiful medical spa for $250. Refreshments, light bites, set-up and clean-up are all included.

• We are excited to celebrate with you and look forward to pampering you and your guests with exemplary treatments, special event pricing, raffles, and giveaways for all!
